FDA Adds Veozah Liver Damage Warnings in “Black Box”

In December 2024, the FDA added a “Black Box” warning label about the potential risk of liver damage from Veozah. The new label warns doctors and patients about the risk of rare but serious liver injuries.

This new “Black Box” warning follows an FDA Safety Communication about Veozah liver problems in September 2024. The FDA urged patients to immediately stop Veozah if they develop symptoms of liver damage. These recommendations have been added to the label.

What Should I Do?

Patients should seek medical attention, and get liver testing, if they experience symptoms of liver damage from Veozah:

  • Fatigue (suddenly feeling more tired)
  • Decreased appetite
  • Nausea
  • Vomiting
  • Itching
  • Yellow skin or eyes (jaundice)
  • Light-colored feces
  • Dark urine
  • Abdominal pain

What is Veozah?

Veozah® (fezolinetant) is a non-hormonal medication that is prescribed to manage some of the more uncomfortable symptoms of menopause, such as hot flashes and night sweats. Veozah is manufactured by Astellas Pharma and it was approved in May 2023.

How Does Veozah Work?

Veozah is a first-of-its-kind type of medication that targets the root cause of hot flashes and night sweats. It works by blocking Neurokinin B (NKB), which is a brain chemical that controls body temperature.

What Does It Do?

Veozah has quickly gained popularity for women going through menopause, which is the time when menstrual periods become irregular and eventually stop.

Menopausal women commonly experience Vasomotor Symptoms (VSM) like hot flashes and night sweats, which range in severity from uncomfortable to debilitating — especially when they disrupt sleep. They are caused by an imbalance between estrogen and a brain chemical called Neurokinin B (NKB).

What Are Hot Flashes?

Hot flashes can cause a sudden feeling of warmth in the body, which is usually most intense in the upper body or face. Women may also experience sweating, reddening of the skin, faster heart rate, and anxiety.

Hot flashes can last for up to 5 minutes and may be followed by a chilled feeling. This can be uncomfortable and disrupt sleep at night. Many women experience hot flashes daily or nightly for at least 7 years, on average, but sometimes a decade or more. This can decrease quality of life and cause significant long-term disruptions to healthy sleeping patterns.

Rare But Serious Liver Damage After Starting Veozah

The Prescribing Information for Veozah explains that post-marketing data discovered rare but serious reports of women who suffered liver damage. These injuries occurred within 40 days of starting Veozah, but patients can develop liver injuries at any time during treatment.

For most patients, the liver damage symptoms gradually resolved after they stopped Veozah. However, there is a potential risk of long-term liver damage, especially for patients who do not stop taking Veozah.

According to the FDA warning in September 2024:

“If there are signs and symptoms suggesting liver injury, stopping the medicine could prevent worsening liver injury and potentially return liver function to normal.”

FDA Recommends Blood Tests to Check Liver Function

Due to the serious risk, the FDA is recommending that doctors order blood work to check the patient’s baseline liver function before starting Veozah. This will also screen patients for pre-existing liver injuries.

The FDA also recommended follow-up blood work, with liver laboratory testing every month for the first 3 months of treatment with Veozah, followed by tests at 6 months and 9 months afterward.

Patients should stop taking Veozah if blood tests show elevated liver enzymes, and perform follow-up tests until liver problems get better.

Patients should also be informed about the risk of liver damage from Veozah, what symptoms to look for, and when to seek medical care.
